Couple points: The pain doesn't necessarily have a thing to do with obesity (most of your weight is below that area). Spurling Sign: Yes. Clasically, looking up and toward the side of symptoms will elicit a shooting pain into the arm suggesting a pinched nerve in the neck. Less commonly, if the spur/disc is more central, there may also be symptoms affecting the same leg. Again, this would imply a combination of central and lateral pressure. Need to address ASAP: Need to address what might harm you the most quickly. Your chest pain radiating down your left arm describes a heart attack. Take aspirin if not allergic and seek help NOW. Your back and leg symptoms sound more like a herniated disc /sciatic nerve injury. Only after you are evaluated for your heart, a workup for back problems as mentioned should be initiated. Some ideas: It may be you were responding to another medical provider. If not- consider- is there a blood clot in one of the veins? This is an unusual but important cause of these symptoms. Sounds like nerve: Most likely condition would be a pinched nerve. The nerves that supply the feet stem from nerve branches traveling from the leg. The "pinch" could be from the back at its root, but it an also be locally due to musculature. Another question might involve a process such as multiple sclerosis, but that too cannot be confirmed in absence of MRI images. Spinal fluid analysis might be useful.
