Cubital tunnel syn.: Though it can be neck or shoulder in origin this most likely represents a pinched nerve at your elbow. Try not leaning on your elbow and wrapping it in extension at night. Could be shoulder v : Could be shoulder or neck problem. Both can present with same symptoms. This can be shoulder issue with some instability causing nerve symptoms into arm with the pain centered in shoulder or a lateral cervical disc herniation creating shoulder pain and nerve symptoms down the arm or a combination of both! a thorough evualuation by exam & appropriate imaging is needed. I concur with your diagnosis that some nerve compression is going on, and it may be at the elbow, but even stabilizing the ulnar nerve (suppliers of the 4th and 5th digits) at the wrist could help.
