I take nucynta (tapentadol) and percocet for pain. I recently failed two drug tests because nothing registered in my urine. I take them every day. How is this poss?

I take 100mg of Nucynta three times a day. Just now, I wasn’t paying attention and took a double dose. Is this life threatening?
I took 1mg of lorazepam this evening, and about two hours later, my 100mg dose of Nucynta. Safe? Feel ok but nervous I took too close together.
