Not yet known: The exact mechanism of action is apparently not yet known but it just thought to be by the low doses of Amitriptyline being used blocking pain signals in the spinal cord by the increase of norepinephrine and serotonin into the spinal cord tracks that carry pain signals to the brain, best wishes.

Avoid neck extension: With a disc protrusion at the c6/7 level, too much extension of the neck, like when you look up, can make pain and numbness worse. Avoid exercises where you have to extend your neck for any prolonged period of time. Best thing is to maintain good spine alignment and posture and work on the core muscles.

OA: Nsaids are very effective for joint pain caused by osteoarthritis and other types of arthritis. It can also be used to increase the effectiveness of narcotics.

Had once-off prescription oxycodone for severe headaches. Worked well. Depleted, can't afford another rx. Asa causes diarrhea. Pcm not work. Ulcer, no nsaids.

Opiates no go: Chronic opiate use is not recommended for headaches. You need to see a neurologist for a through discussion of your treatment options. There are wide range of meds available depending on type of headache.

Stomach: Toradol is just a strong nsaid; try tylenol (acetaminophen) with aleve, they work by differing mechanisms. If you have chronic pain problems you may benefit from a low dose of suboxone. Pain issues can be pert of a spectrum of mood problems and may be linked to addiction and mood. Also the snri class of antidepressant is good for pain. Cymbalta at 30 mg is not addictive and indicated for arthritis. Good luck.

I know there's no such thing as a "safe nsaid", but what would you recommend for long term usage for a pt w/ gerd?

No specific: All nsaids can cause damage to the GI tract. If you require long-term use of nsaids, then discuss with your prescribing doctor how to best protect your GI tract. This is not a forum to advise on specific medications, as such advice requires knowledge of the entire medical history of a patient.

See below: All medications are potentially dangerous. You only take them considering risks and benefits. Nsaid's are known to cause kidney and liver problems and their effect should be monitored. If your medical problem requires the therapy with no other good options, you take the medication.

Many complications: The commonest are GI upset, heartburn and some blood loss from the GI tract. They reduce the efficacy of blood pressure meds. They affect the kidney and may cause protein in the urine even in young people. As already mentioned, they prevent the beneficial effect of Aspirin on heart disease. Asthmatis, who have sinusitis or nasal polyps and Aspirin allergy, they may develop severe asthma attacks.
