Difficult to say but: It's possible that your chest pain could be gastrointestinal related like reflux. At night, it's not uncommon that stomach contents can "reflux" into your throat. Reflux into your food pipe(esophagus) can cause the chest pains you describe. You should review this possibility with your primary care and in the meanwhile consider sleeping with several pillows or place your bed at an incline. ...Read more

Esophagus?: I wonder if you don't have something wrong with your esophagus, the food pipe that connects your mouth to your stomach? These pains frequently occur more often at night, and would be missed if only a chest X-ray and EKG were run. Typical workup: Yes, it does sound like they did a very typical and expected workup for your complaint of chest pain. While I am glad your tests came back normal, i know still feeling the pain must be frustrating. Have your doctor consider costochondritis, which is inflammation of the cartilage between your ribs. Pains/aches: Not sure what is wrong but strongly suggest you get a referral to a heart specialist.
Till proven otherwise something is wrong.

4 days of intermittent stabbing chest pains. On right side of sternum, sometimes left. Sometimes in my throat. No SOB. Went to dr, they gave me nexium (esomeprazole). No EKG. No chest X-ray (Do I need one?)

Depends: X-ray would only show abnormalities in some conditions like pneumonia or pneumothorax. In other conditions like reflux it will be normal. The doctor would order the exam if they think an X-ray could answer the question. ...Read more

Possibly serious: It is possible that this pain could be serious. The most concerning possibility would be a blood clot in your lung. This would cause a feeling of shortness of breath. This is often not seen on a plain chest xray. You would need to go to the emergency room to be seen for this. The other likely less serious cause is a muscle pull. Consider stress test: If the chest pain comes with effort, normal ECG and blood work when not having pain does not exclude angina. If done while having pain, much more helpful. Wow: Do you smoke? Obviously if you don't smoke something else is going on. Unusual at age 20 but with your co morbidities especially diabetes i think you should be under the care of a cardiologist ; an endocrinologist and make sure they consult with each other. We never treat the blood or other diagnostic tests we treat the patient. Normal tests with continued complaints deserves more diagnosis. ...Read more
