For elevated ammonia lactolose is used as is rifaximin.. it is important ot take medications. With elevated ammonia you get confused, disoriented, slurrign of speech ultimately lethargic when very high can result in coma. Hydration and low protein diet is important as well

Ammonia Smell: The most likely cause is a change in your intestinal bacterial flora. However, is it possible that the smell is actually from your urine? You can easily check. Why an issue?: Not clear why you would have had a blood ammonia level, as this may be quite misleading if your liver studies are in normal range. Not enough info: Have the gallstone & past pancreatitis issues been treated with gallbladder surgery? What radiology tests gave been done? Is there bile duct blockage? Need more info: Hepatic encephalopathy is diagnosed by mental status. If he is confused, disoriented, or abnormally sleepy, probably he has it. High ammonia can help confirm encephalopathy, but by itself doesn't make the diagnosis. However, that plus not taking lactulose probably mean he is at high risk for it. Check with his doctor ASAP. All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Having said that, your level is nearly twice the normal level. See this site for more info.
