Isotretinoin: Just be patient; it is normal to have some slight flaring in first 2 months. Do not pick at the lesions, it will just make them worse. Most patients look pretty great after 3 months. Read more

Cystic acne: Serious acne, that is the cystic type, should be treated with Isotretinoin at a measured dose depending on your bodyweight. If you have painful cyst, your doctor can inject a mild form of cortizone (hydrocortisone) which will calm them down fairly rapidly. Definitely keep taking medicine that she or he tells you, and attend the follow-up sessions faithfully and stay on the medicine for at least 5 to 6 months. Read more

Hormonal Acne: If you are having acne breakouts during menses while on isotretinoin, your problem is likely more related to hormones. I would see your doctor about a hormonal workup (check testosterone, fsh/lh, dhea-s), and look at more hormone specific treatments for acne, like spironolactone, cyproterone, or flutamide. Read more

Serious Rx: If you have not been offered a trial of isotretinoin, please get with your physician now and ask about this. It is not for the faint-hearted and there are risks, but most people who have needed it are glad. You also want an endocrine evaluation, and you may consider the oral contraceptive pill if it is right for you. Laser isn't a magic answer. Read more

Under dosed?: See your dermatologist and ask him if the 30 mg dose is sufficient. It is usually based on weight. The standard is 1mg/kg/d so a 130 pound person should be on 60 mg per day. Also, it is not unusual for people to get a little worse on Isotretinoin before they get better- so don't panic! Read more

This is serious: I'm glad you're on isotretinoin and this is appropriate for a woman in your situation. Hopefully you'll get a good result. Other meds that may help if you relapse include spironolactone and the oral contraceptive pill. Lasers may be an adjunct for you.
