Not herpes: No herpes outbreak is likely to last 3 weeks, it's an uncommon cause of balanitis. If herpes were the cause, it would be in the form of open sores, generally very painful. Yeast indeed can cause balanitis. Maybe share your gf's anti-yeast cream (or buy clotrimazole cream over the counter). Also retract foreskin and wash w/ soap and water 2-3 times a day. BV? STD?: Contrary to what some people say, vaginal yeast infections usually do not cause discharge with odor. The main yeast symptom is itching/irritation. Bacterial vaginosis, an overgrown of vaginal bacteria, is a common cause of discharge with odor, but STDs (gonorrhea, chlamydia, trichoonas) also can do it and could cause positive urine nitrite and LEU. Trich? BV?: With "all STD tests negative", still ask your doctor if you had the newer trichomonas DNA test, much more accurate than older tests but not always done. Also ask about bacterial vaginosis (although BV doesn't usually cause colored discharge). Other non-STD vaginal infections also are possible.
