Normal variant?: It's possible that it could be a narrowing of the vein between there and the heart, or a clot in the vein, but either of those would be extremely unlikely in a healthy 20 year old man without a significant surgical history. Not: Just enlarged vein is not a problem so long it does not have a clot in it. Also the flow pattern needs to be defined. If the flow is slow that could imply blockage somewhere else. Consult with your physician. ...Read more

Positional change.: The veins in the neck just as the veins in the rest of the body change in size depending on changes in position as well as changes in levels of hydration and changes in pressure in the chest and abdominal cavity. Unlikely: The veins usually don't undergo dissection but the arteries do. However it would be highly unlikely,if not impossible , for dissection to occur with hard coughing. But a few flecks of blood from broker capillaries in tge bronchial tree may be associated with heavy coughing. ...Read more

Pinched Nerve/Muscle: Most times back pain that is sudden onset will resolve. He should use anti-inflammatory meds like ibuprofen, aleve around the clock and even tylenol (acetaminophen) to help. If this does not resolve you should see your doctor for a muscle relaxant as well. If it still does not resolve, I would recommend seeing a spine specialist for a full evaluation and testing. You may need to get an MRI to see inside. ...Read more

R neck / parathesia: Is the numbness only on the right side or both sides? Would get a carotid doppler on the neck, check for diabetes, TSH/ T4, B12, magnesium, lyme if you had a toxic exposure lead/ arsenic test. Consider MRI of head and neck if all else is negative .
