TMJ: This probably is not sinus issues but temporal mandibular pain from the joint. Take Motrin 600 to 800 mg three times a day. Use warm compresses and eat a soft diet: mush the consistency of yogurt until the pain stops. Stop chewing gum or eating hard foods like bread and meat that require a lot of chewing. Hypertension?: I notice you are on pain meds, but I don't see that you are on anything for high blood pressure which you say you have. This sounds like neck muscle spasms. It is very common and nothing to worry about. I suggest using a heating pad every 2-3hrs, massaging the pressure area, and take over the count pain medications such as tylenol (acetaminophen) or naproxen.
