- reflux disease
- sleep apnea or other sleep disorders
- asthma, cough variant asthma
- possible chf
i would start with complete pulmonary function tests, cxr, and then a sleep study with empirical treatment for reflux disease for a few weeks. If you smoke or consume alcohol, I strongly recommend to stop these.

You are right: You don't want to take an antibiotic unnecessarily, unless directed by your doctor, all what you have can just be a viral infection.
