Not ER but...: A temperature of 99.8 taken rectally in a 3 week old is normal, so that wouldn't be my concern; however, the symptoms of cough and chest congestion warrant a visit to see his pediatrician this morning! He is young and those symptoms could be indicative of RSV, flu, or other illnesses that should be evaluated. Could also be reflux. Babies can normally sound congested but not with a cough.
