See a dentist: I can't tell you if you need a root canal to save your tooth or if it is too badly broken to treat it. If you can save your tooth with a root canal then get it. Get the tooth checkd: The concerning part here is the past history of a root canal. Many upper molar teeth commonly have complexities that contribute to a later infection. This infection can often spread to the sinus, so the 2 could be linked. I would not take an antibiotic without first being evaluated. An endodontist (preferrably one with a ct scanner) and that uses a micrscope would be best able to assess you. ...Read more

I : I assume that your root canal is completed. It is not unusual to have some pain after completion of the treatment due to residual inflammation of the tissues around the root of the tooth. This can be managed with Motrin or advil. I may also be present if there was an infection in the bone as a result of the death of the nerve. Multiple causes?: It is possible that (1) the root canal treatment is not fully completed. (2) it is "completed" but a canal was "missed" and a nerve is still signaling hot / cold sensitivities.
