Likely bursitis: Sounds like inflammation of the prepatellar bursa (flattened sac that provides protective layer between knee and skin). Usually a result of prolonged kneeling or other "wear-and-tear". Rest (avoid kneeling) and sometimes a knee wrap for protection is helpful. If no contraindications, can use nsaids for inflammation. Infrapatellar bursitis will frequently complain of pain and swelling in the front knee below the patella that can radiate below the knee often, is causes severe pain with kneeling or walking down stairs. I have a burning, stinging severe pain on the outside of my left knee when kneeling...even when climbing onto my soft mattress.

Iliotibial band: could be inflammation of distal iliotibial band, could be knee arthritis or meniscal tear. I would start with NSAID treatment, barring no contraindications (ulcer, kidney disease). I would at least obtain an X ray of the knee and trial of physical therapy. You can seek interventional pain medicine, and seek injections.
