See Your Physician: Most back pain is self limiting but if your pain is that severe, you need to evaluated by your primary care physician. There are many causes of back pain and without a history and physical examination, it is impossible to determine the cause of your problem.

Get it checked: Hard to say what it is but if it effects your breathing it should be checked.

Several Possibilities: This likely is a simple back strain as upper back muscles do expand during breathing and if strained, will cause pain. However, if you have chest pain, shortness of breath and new swelling to one of your legs, take birth control/smoke, have a history of cancer, a recent surgery -- pulmonary embolism while unlikely, is a possibility. M.D. visit: The pain may be related to a number of conditions. If you are experiencing worsening pain, despite Ibuprofen to treat muscular strain, then a visit with your physician is recommended. They will access you for conditions such as asthma, aspiration, pneumonia and heart disease. Noting what makes the pain worse or better will be important information for your physician.

Possible spasm: Of infrascapular muscle. If symtoms persist- see your physician. Place tennis balls in a sock and place under te sore muscle. Advil and Aleve (naproxen) also work if not allergic to these products.

? pleurisy: It may be an inflammation of the lining of the lung. It can cause pain when you try to take a deep breath. Are you having any chest pain, cough, fever? It is important to determine the underlying cause. Go to the emergency department of the nearest hospital. Pleurisy may not be the diagnosis and a physician needs to evaluate you to make sure.

Maybe more: This maybe more than heartburn, you may have a gallstone issue or an actual ulcer. See an MD for an exam and likely an ultrasound of your abdomen and maybe even a chest X-ray.

No: It is muscle spasm due to wrong posture, bedding or pillows. Try to correct that and wait for 1 week. In meantime do brisk walking for 1 hr. May take some warm shower and NSAIDs. don't do any bending or stretching until pain goes away.

Cough and back pain: Symptoms of back pain, cough and pain with breathing suggests a respiratory or cardiac cause that could be potentially serious. The best approach is to see your healthcare provider and be evaluated for cardiac and respiratory disorders. I suspect a chest x-ray, ecg, and lung function tests would be a fairly routine place to start. Once identified, specific treatment can be given.

Multiple possibiliti: This could be related to muscles and joints from arthritis type problems. If you have pain shooting down your leg + back pain, you may have some disc problems in your lower back in addition to the upper back with your complaints. Pleurisy may do this and can be caused by a number of different problems. More info is needed to answer fully. See a dr again if this persists or worsens. Lose weigh.

Dermatomal?: Although I agree with the previous answers would also add "shingles" to the possibilities. It can give pain around the rib areas on one side and may or may not be directly associated with a rash. Can be quite painful but there are treatments available. Important to see your doctor to help determine the next step for treatment and work up.

See a Dr: A month of even moderate pain is long enough to wait. Extreme = DO IT NOW. You need to be examined. It might be nothing major, but let a pro look at the situation. Breathing and moving are not optional. The cough might need attention too. It's worth it - you are. Get insurance if you need to - it's why the ACA exists.

Did you pull a muscl: Doing some unusual exercise or activity before this started? If so, it is probably safe to apply heat and wait it out. However, if you did nothing unusual and you have pain on deep breathing it could be something more serious like a collapsed lung or blood clot and you should be seen. Good luck!

Poor posture: It could be from poor posture. If you sit a lot, you could have slouched shoulders and neck which pull on your shoulder blade. You could have a shoulder-neck strain syndrome. This can be improved wiht stretches of your neck daily, improving your posture and/or massage.

Sharp pain: Some times is very difficult to give advise without examining the patient. Therefore, my advise is to see u pcp for evaluation and work up.
