Given status, angina: is a high possibility. Good that you feel better with exertion but a large heart attack would dramatically change your capability & most are asymptomatic. Study my answers, optimize NMR lipoproteins (LDL <400-700 nmol/L, Large-HDL >9 ?mol/L, https://goo.gl/1nvgER ), HbA1c to <5.0%, usual sitting Systolic BP <120 mmHg. Yes: It is quite common for cough and wheezing to increase after smoking cessation. The hair cells that line the resp. Tract wake up and start working, moving all the junk that you have accumulated in your airways. This can go on for several weeks, but tends to improve within a month. You need to see MD: Although you are young, your symptoms are very alarming and definitely not normal. I would suggest you follow up with your PCP very soon. The diagnosis can range from mild causes starting by anemia to more serious that involve your heart or lung. Again, my advise is to see MD. I hope you will feel better. ...Read more

Don't worry: I'm sorry you've had a bad cough for 6 days. No need to worry about the ear popping back. As the congestion in your upper respiratory system resolves the pressure in your eustachian tube will come back to normal on its own. You don't need to do anything. Don't try to "pop it back".
