Tuberculosis: I'm not sure why, but from comments this forum I have the impression that in the Philippines, tuberculosis is often called Koch's infection. This chest Xray means the person has TB. YES, SAFE: Zithromycin, unless allergic is a good choice for a wide spectrum antibiotic, very commonly used for upper respiratory infections. In addition 500 mg is a quite large dosage. Antibiotics take some time to work. Your wife should just now be turning the corner if it is a bacterial infection suited to this antibiotic. If not, it may be viral or needs a different antibiotic. R/O PERICARDITIS: You may have inflammation of the heart=PERICARDITIS vs inflammation of the lungs =PLEURITIS. Myocarditis is much more serious and can be suggested by an abnormal echo. Often they are post viral.They tend to resolve with rest/hydration /NSAID(indocin) if it persists as course of prednisone may be needed. You need a prompt evaluation EKG/CXR AND LABS.
