Neck/back pain: Lung cancer is unlikely. The symptoms are more likely due to muscle/tendon strain or a disc herniation in the neck or mid back area. Difficult to say: small shadow can be many different things. In the absence of old studies this could have been there for years. Old study is crucial before jumping to conclusions.
