Doubt melanoma but..: If you have the least concern of a melanoma, it needs to be seen. I suspect instead that you picked off a seborrheic keratosis -- melanomas really can't be picked off. Show your physician the site next time you have an office visit, or if a suspicious lesion recurs, get seen right away. ...Read more

a=asymmetry of lesion
b=border is irregular
c=colors are multiple and not consistent
d=diameter is greater than 6mm

Take a picture: If a mole becomes larger, irregular, changes color, bleeds, develops a ulcer or a nodule, it would be important to have it checked out and if needed removed. Trauma?: Do you remember any trauma to the toenail? A collection of blood underneath the toenail can also appear like this. The safest thing to do is to show this to your friendly dermatologist. While it would be rare of a 20 year old to have a melanoma, it does happen occasionally.
