What are goals?: The answer depends on your long term goals. If you do not want more children than an endometrial ablation is a good option. Assuming you do want more children than other options can be considered. Testing for PCOS and hypothyroidism could be done. Since you are missing periods followed by heavy bleeding PCOS seems likely. A sonohystogram should be done to see if a polyp or submucosal fibroid is present. If present then these can be surgically removed as an outpatient. Subclinical Hypothyroididsm: In a difficult case like yours I take into consideration ratio of T4 to TSH. If the ratio is less than 2.5 I have found out that patients have hypothyroidism. I would take small dose of synthyroid like 0.25mg for a month and recheck T4and TSH then decide whether to continue or add some more. This will depend on how you tolerate it and what's your T4 and TSH. Obviously you have compound problem in situation like this hypothyroidism needs to be corrected first.
