Usually starts L-->R: At birth the left side has higher pressures than the RT side. Thus flow is mainly L-->R. If the ASD is small it may close on its own but if not tends not to cause a problem. If it is large the flow L-->R increases volume load on the Right leading to increased RT pressure, this will reverse the net flow to Rt-->Lft. Echo for PFO: An echo is done with the ultrasound probe on the chest wall. The quality of the image depends on the "window" between your ribs. It is usually not easy to see the atrial septum well. Transesophageal echo (TEE) with the probe in the esophagus gives much more detailed images. One question, though, is whether TEE would change anything about plans for your treatment. The link above gives good information on risk and diagnosis of Atrial Septal Aneurysm. The big issue is when blood flow across the septum. Clots from the legs are usually stopped in the lungs. With a ASD, the clot will cross over and go up to the brain can cause stroke.
