2 TB skin tests. Bumps measured 15cm. RN stated I possibly have LTBI & ordered QuantiFERON Gold in-tube test. Could anything cause a false positive?
Not at 15cm, but...: The bump is more likely 15mm, because 15cm is 6 inches and HUGE. A 15mm bump on a screening test is big but may still be a false positive (a screening test means the patient has NO symptoms of tuberculosis). If a patient has contacted somebody with TB, or has current symptoms, she likely has a true positive unless her bump is due to past BCG vaccine. Is ELISA test for HIV 1 & 2 antibodies conclusive after 8-9 weeks of exposure? Does NON-REACTIVE mean NEGATIVE?
ELISA for HIV: The ELISA test is not conclusive after any amount of time. The ELISA is a screening test and if it comes back positive, a Western blot will need to be done. Non-reactive is a good thing to be and it does mean that you are negative for HIV virus. Although there is a window period when you can have the HIV virus but still have a nonreactive result. Hep C unlikely: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Viral exposure: Virus antibodies by IgG suggest prior exosure. If active disease the test will be positive by IgM. So previously you have had rubella and cytomegalovirus (CMV) infections. The latter can cause hepatitis. If you have liver disease you may need a DNA probe to check for viral load. Probably yes, but...: I'm not familiar with HSV blood tests that give results as fractions or titers. With other tests (e.g. HerpeSelect®), any positive HSV1 result usually is valid, but weakly positive results (which is what I suspect is how to interpret your HSV2 result) often are false and require confirmation. Sorry - wrong track: I'm concerned we are on the 'wrong track' here. CMV is not really a major problem for a 'normal' patient. I think your lab testing and evaluation should concentrate on different possibilities. Have you had muscle enzyme testing? Thyroid? Lyme? Rheumatological tests? Evaluation of medications? Almost everyone has been exposed to CMV -so it's no wonder your IgG is positive. IgM would have no value. ...Read more

No...actually: The zn stain is a broad stain for acid fast bacilli of which mycobacterium tuberculosis is one of many. The specific tests like quantiferon or the pcr test are against you having tb. The culture is the "gold standard" but you will need to wait for it. It might turn out be a cousin of TB and that too will grow out of culture.
