50+: Many men remain fertile well into their 50s and 60s. However, the risk for conceiving a child with some kinds of uncommon genetic disorders increases when the man is more than age 50.

It doesn't (so far): Avastin (bevacizumab) is a monoclonal antibody that inhibits blood vessel growth. Is the OTC male fertility test accurate?

Yes. Not a full test: The over-the-counter fertility test for men is called spermcheck and tests only sperm count (concentration): low or normal. Accuracy is ok. A full semen analysis (sa) through a fertility clinic looks at sperm count as well as motility (% that are moving) and morphology (% of normal-shaped sperm). Some clinics test sperm antibodies too. The home test is better than nothing but is not a full sa test.

Semen Eval & bloods: The basic test is the semen analysis which is typically collected by masturbation into a sterile cup. This is best done in a specialized reproductive lab, as commercially done analyses are often not very reliable. Hormonal evaluation of testicular and pituitary hormones is also done, and in many cases a special physical examination to evaluate the testicles and scrotal sac (for varicose veins).

PSA: Psa is produced by the prostate, it can be elevated due benign increase in size of the prostate, prostate infection, and cancer of the prostate. Infertility is not usually caused by an elevated psa. Prostatitis could theoretically be a cause but usually on a temporary basis.

Steroid side effects: Even high dose solumedrol (methylprednisolone) is not known to cause infertility. Other immunosuppressives can cause infertility. Why do you receive this treatment? The underlying cause or disease process you might have could also cause this. Need to check in with the doctor giving you solumedrol (methylprednisolone).

Probably not: Drugs have short-term effects on male fertility through suppression of sperm production. Long-term effects would reflect injury to the testes, the sperm producing organs, and such injury is rare. I would search elsewhere for a source of the problem.

See urologist: Management of "male infertility" depends on the nature and etiology of male infertility. For example, mildly decreased sperm count or severe (no sperm). There are several causes for having a low sperm count. Best to seek a urological evaluation to possibly determine the cause of an abnormal semen analysis. The etiology will determine the best option to correct the problem, if possible.

Semen analysis: Semen is the fluid associated with the ejaculate. Semen contains sperm. Sperm can be counted and the quality assessed. These are the primary way to objectively assess fertility in a male. Other factors are important: chromosomes, presence / absence of certain sugars in the ejaculate, presence /absence of infection in the ejaculate; retrograde flow of sperm to the bladder can be a culprit.

Recent evidence, yes: Recent genome and population studies associate paternal age with autism, schizophrenia, and possibly cancer. Mice studies show decreased fertility with middle age and altered sperm genes. An english study showed that men>45 years needed 5x longer to achieve pregnancy with partner. Male age particularly becomes a factor when women are in late 30's. With ivf, pregnancy rates are less for men >50.

Many: Lack of sperm or azospermia can be due to either obstructive causes in the tract, from the testicle to the urethra, or from non-obstructive causes such as primary testicular dysfunction due to infection (mumps), history of surgery / trauma / chemo / radiation, or idiopathic (unknown). The biopsy should guide the next step in therapy as well as a hormonal panel, particularly fsh, sex chromosome.
