My 3 yr old has a very high neutrophil count, very low lymphocyte count, high sed rate, low hematocrit. What could be going on with him. Repeat labs?
Infection: The shift to neutrophils and away from lymphocytes occurs during the early stages of fighting an infection or fighting a bacterial infection. The sed rate is a nonspecific marker, that could be up w/infection, or lots of other things. The low hct doesn't fit well, unless just barely out of normal. Lots of B cells: A lymphocyte enumeration is a count of the lymphocytes in your blood. Lymphocytes primarily include t-cells, b-cells, and nk-cells. B-cells are identified by cd19, a marker on their surface. Have the doctor who ordered the test help you interpret the result. Viral: Usually if we do a blood count for a person with an infection would look at the total white count as well as the differential. A high white count with a lotto neutrophils suggest bacterial infection whereas a white count with a lot of lymphocytes is more suggestive of ireland flexion. It still could be ap: It still could be appendicitis but the lymphocytes suggest maybe a viral infection. It's just you: Reference ranges are set so that several percent of healthies fall outside on either side. If you're older and actually seem sick and the absolute count's more than say 8000, a pathologist might check you out for preleukemia, though it's not really treatable. Unclear: Lymphocyte "count" usually refers to blood. In tissues lymphocytes can be benign and due to inflammation or autoimmune disease, but can also be malignant (not as common, especially in someone so young). Insufficient info: Your husband may be anemic and needs further evaluation for the cause. A high mean corpuscular hemoglobin concentration has little meaning absent a full picture of all other parameters measurable. The high % of lymphocytes, again, in and by itself, has little impact on establishment of a diagnosis. No: I'm confident here. Lyme disease is both a real entity and a "pop" / "fad" diagnosis. There are normally a lot of lymphocytes in the small intestinal mucosa, though having a lot in the epithelium itself is more suggestive of gluten enteropathy or a few other entities. Must ask physician: It's key that you realize that reference ranges on lab work are set so that a few percent of healthies fall outside on either end. If you have HIV-negative and feel well, low lymphocytes seldom mean anything. White cell percentages never mean anything apart from absolute values, and high neutrophils can result from something as simple as a cold shower. ...Read more
