See details: It is a test for lyme disease. A positive screen alone is not diagnostic. It needs to be confirmed by the western blot test which actually tests for specific antibodies related to the lyme organism.

Yes, positive 20+ yrs: Research suggests igg antibodies can persist at least 20 years after infection. While igm antibodies appear first, unlike other infections they often persist ; are a good indicator of persistent infection. One may have chronic lyme with or without persistence of igg antibodies. Chronic lyme is difficult to fully eradicate ; no current test proves that it is no longer present in your body!

Then it's not Lyme: Lyme disease can be tricky -and you want to make sure you see someone like an Infectious Diseases doctor who knows how to treat it. The diagnosis is actually based on clinical findings, and the lab testing is generally 'supportive'. Since you're Western blot is negative, then it's very unlikely you have Lyme. There are other diseases that could cause 'Lyme-like' symptoms -and good to get evaluated

Yes: The presence of IgG usually indicates past infection which might have occurred months or even years ago. The presence of IgM indicates a current or recent infection.
Old Infection: This most likely means you had an old infection with lyme disease. Our bodies make IgM early in an infection and IgG later. Even once the infection is gone there will be detectable IgG for quite some time. This is part of how our body remembers previous infections so we can fight them off better if we get them again.

Not necessarily: IgM antibody means recent infection. IgG antibody usually means remote infection. You may not have the disease, just that you may have been infected at one time.
What to do if I did a Lyme disease test and it showed IgG p58 ab. Positive a can you explain what that means?
Nothing by itself: A single positive IgG band means nothing. Was the screen positive? How many other IgG and IgM bands were positive? You should discuss all of this with the doctor who ordered the test.
May be related to it: It could be related to your inflamation - especially any vascular issues. This will depend on the manfestations that you are having. You can discuss that with your physician.
