Throat tightening: First one needs to find out if you are tightening up your throat when you are breathing out and thus the whistle. Non-specific: Fleeting chest pains are not specific for lung cancer. It could be caused by a host of other benign reasons like muscle pain, rib pain from strain or inflammation. Not cancer: Congratulations on your smoking cessation. Lung cancer doesn't present this way. The pain is almost certainly chest wall pain, but if you are really short of breath as well (and it's not just from discomfort), you need a competent fitness evaluation and perhaps spirometry in addition to your CXR and exam. PFT work up: It is common to have a cough after most of the other symptoms of a viral illness have resolved. The shortness of breath though should not occur in association with the cough.
