I have whistling when exhaling. Doctor said lungs sound clear and the whistling is heard over the neck. X rays, laryngoscopy, PFT all normal. Causes?
Throat tightening: First one needs to find out if you are tightening up your throat when you are breathing out and thus the whistle. When this happens next time, relax your lower jaw to see if the whistling disappears. Although the PFT was normal, was it obtained at the time you had the whistling? Can reflux cause wheezing? Peak flow 600. Albuterol doesn't seem to make it go away, just reduces the volume a little. X rays okay. Lungs sound clear.
Reflux & wheeze.: With GERD, especially with LPR, the refluxate can ascend from the esophagus into the pharynx, and then down into the airway causing inflammation and, eventually, a wheeze. Not cancer: Congratulations on your smoking cessation. Lung cancer doesn't present this way. The pain is almost certainly chest wall pain, but if you are really short of breath as well (and it's not just from discomfort), you need a competent fitness evaluation and perhaps spirometry in addition to your CXR and exam. Adult-onset asthma might be the explanation; it's manageable. Best wishes. ...Read more

This may represent bronchospasm or another cause. I would advise a pulmonary function test. If your lungs are clear and you are not febrile, pneumonia is less likely. ...Read more
