Epididymitis can also be very painful & would require antibiotic treatment. Some degree of pain relief, by gently lifting scrotum by hand, would suggest epididymitis & increase pain if torsion. Wart sounds unlikely: Genital warts are most common on the penis, not so common on the skin of the scrotum. Also, single genital warts are uncommon; usually there are several.
