Depends: How long has it been there? A few days, weeks or months? Is it painful. Did you have a recent injury, cut or infection further down that arm in the past few days. Watch it closely. It could be: Any of the three you mentioned but most likely folliculitis. The gland at the root of the hair is clogged and is not able to express the secretion and swell up and get infected.
