Could i mistake a ruptured spleen with an intercostal sprain? The pain is on the left side and back near the "floating" ribs. No blunt trauma at all.

Many things: The organs there include stomach, spleen and colon in front and kidneys a little lower in back. Even cardiac issues can present rarely in that area. Could be anything from an ulcer to kidney stones, hard to tell without physical exam.
