Xray: I would maybe suggest he go be seen by his/a doctor and have an X-ray of the hip and rule out a septic joint. You don't mention him having a fever which would make me think that even more, but could be early in the game. Let's say the pain occurs all the time. It may be the physio is not helping. In such a case, an MRI of the lumbar spine or EMG test may help determine what's wrong.
Perhaps the problem is worse with movement. That may indicate a need to see an orthopedic specialist.
