severity of pain of 8/10,I suggest you seek medical help
preferably see a urologist.
As for your low back pain, this is to be taken care of after tending to your testicular pain.
I do not know what you do, is your work exertional?

Arthritis/stenosis: This could just be some arthritis of the back. In an acute injury it is technically better to use ice. If no injury, then which ever of ice or heat helps.
