Monthly?: Many gyne issues, endometriosis, menstrual cramps, ovarian cysts will cause those symptoms (predictably) monthly. Pregnancy can cause that. Based on the whole constellation of symptoms though, I would think that you are having problems with you gallbladder. The symptom of the pain getting worse after eating, is what tips me to thinking it is your gallbladder, Especially if you are eating greasy and / or spicy food frequently.

Multiple causes.: Kidney infection, kidney stones, old menstrual blood mixed with urine, sexual trauma, urethritis, etc. If this continues recheck the urine; however, I suggest a helical CT scan of the kidneys to look for a stone as well as a pelvic ultrasound.
