As a general rule of thumb, any pain in the abdominal area that is significant and lasts more than four hours should be evaluated in an emergency room setting. Possibilities include diverticulitis, crohn's disease, irritable bowel syndrome, tumors, infections, polyps, hemorrhoids causing the pain when youdefecate. Ulcerative colitis is also possible.
