Multiple causes: No single illness will explain these. If physical exam and labs are normal, look to a functional cause. Headache can be due to something as trivial as needing eyeglasses. "Clear urine" is not an illness. Look at your entire life situation, including sleep patterns and what difficult interpersonal situations may exist your physician could perhaps help with.
