Total thyroidectomy 8 yrs ago. Nowadays I am gaining weight. Recent labs low TSH 0.362 T3 =90, T4=6.9 Synthroid decreased 125 mcg to 112 mcg.?

TSH may be low simply because your thyroid meds are doing enough to keep the TSH at a low level.

In fact radioactive iodine ablation is effective, with 40-70% of patients becoming hypothyroid after its administration. No significance: T3 (liothyronine) uptake is an old test used to indirectly measure t4. Now that we have assay to accurate measure free t4, we don't use this test anymore. A high TSH with a high T4 is very uncommon.
It is possible, but unlikely that your pituitary gland is producing too much TSH.

Difficult to answer: Since you're on ptu (propylthiouracil) which inhibits thyroid function and hormone production. Would have to know when and how much ptu (propylthiouracil) you've been taking and serial lab values to determine the answer. If your TSH is still suppressed, you're probably still hyperthyroid, but more or longer therapy w/ptu (propylthiouracil) may help. Methimazole may be safer than put. However, the suspected hyperthyroidism needs to be confirmed with high levels of T3 (liothyronine) and free t4. Unlikely but...: Rhodiola is known to improve thyroid function and thus might be expected to reduce TSH levels somewhat (though more research on this is needed). Low TSH unlikely: Thyroid cancer usually does not cause a low TSH (unless you are taking thyroid hormone). Most thyroid cancers do not function as well as normal thyroid tissue. Elevated thyroglobulin is a nonspecific finding, seen in most disorders of the thyroid. It can serve as a tumor marker for recurrent thyroid cancer as long as the thyroid has been completely removed. Thyroid labs: Hi. See an endocrinologist. Make sure it's a free T4 (not a TOTAL T4). If you have low TSH and a low free T4, that's compatible with pituitary disease causing hypothyroidism (so called "secondary hypothyroidism"). Mild hyperthyroidism: Unless you are taking thyroid medication (and taking too much) this could mean mild hyperthyroidism. If your doc measured TSH, FT3 and FT4, (s)he must have suspected something.
