Although the tests you have had done are negative, it is quite possible that you there are additional tests that would shed light on this. Need more info: If she has really seen a dozen doctors and they have not solved this, there is no way in which a diagnosis can be made without a detailed history, physical exam and review of all prior records. If she really has fevers (temp >100f) and can document the weight loss would suggest that she see an infectious diseases expert. Sounds ill and needs full evaluation. If you have actually had fevers (temps reaching 100f or failing to get as low as 98.6 in 24 hr periods) you have a fever of undetermined origin, and there are dozens or more potential diagnoses, most non-infectious, but the person to see about this would be an infectious diseases expert. Probable hormonal: It is unusual to have a true fever during a menses but not uncommon to have hot flashes and sensitivity in warm environs. This is often called PMS. Possibly a sinusitis with post nasal drainage causing coughing. If so, excessive coughing can cause extra use of ribs and muscles between the ribs, stretching ligaments. This is Costochondritis + can cause stabbing pain when twisting or coughing.
Also could be infection-Bronchitis or even Gall Bladder (pain RU abdomen).

As lng as she has no other complaints (sore throat, ears, hurts when she pees, etc), then you should be ok to watch and wait for 7-10 days.
