Hair follicles don't grow out of scar tissue, but if the scar is narrow, and the surrounding skin is making hair normally, when it grows back, the scar may be barely noticeable. Male pattern thining: This may be a genetic type of hair loss but uncommon in a 20 yo. History is important. If she uses a hair style that pulls the hair tight on the anterior scalp she could have traction alopecia. This could be a patch of alopecia areata which would feel smooth like velvet. The main thing: to differentiate here is whether this is coming from a skin condition such a dryness/seborrheic dermatitis or from a nerve problem such as a superficially compressed nerve near your neck.
