See remark: Chad, your question is not so clear. From what I think you had repeat repair of a hernia which originally took
place 16 yrs ago! You currently may have HYDROCELE
(fluid around the Lt. Testicle).You also mention fluid near incision site!!

Cremaster Muscle: The vas and blood vessels to the testicle are covered by a muscle layer that has to be divided when performing an open inguinal hernia repair, called the cremaster muscle. Depending how extensive the dissection, your testicle may not raise up any more. While I can understand your concern, this should have no major effect on your testicle function. Probably normal: Swelling in the groin, the testicle, and scrotum after inguinal hernia repair is very common and should get better day by day. Try elevation and ice/heat. If the pain is persistent and/or severe or if the swelling doesn't get better there is a small chance of damage to the spermatic cord. You should see your surgeon and monitor closely. ...Read more

Post op swelling: This may be part of normal post op recovery after hernia surgery, of course if its on the same side as the surgery. If your surgeon manipulated the cord and placed mesh some discomfort may be ok at 6 weeks. Sounds normal: One testicle almost always hangs lower than the other; this is normal in all men. (Most commonly the left is lower, but it can go either way.) Normal: It is normal to have some discomfort after any type of hernia repair. The length of time is variable, and there is nothing to worry about if it is minor. Depends: It is not uncommon to have discomfort after an inguinal hernia repair for several weeks. In fact you probably won't feel completely back to normal for 6 to 8 weeks. If the pain is getting worse or not getting better at this point you should be seen by your surgeon for follow-up evaluation. If there is no redness or drainage and the pain is getting better over time it may be safe to watch. ...Read more

Moderate chance: There is of course pain and discomfort after most operations, and Inguinal hernia repair is no exception. The pain however is typically well tolerated, and limited. With a laparoscopic approach, typical recovery times are a few days -2 weeks. With the open approach, 1-3 weeks. Both have their benefits and risks. Questionable treatment, unless indirect inguinal hernia shown first.: Testicular &/or scrotal pain can notoriously be difficult to manage and often does not respond to epididymectomy. Certainly removal of testicle should not have been performed unless testicle was very tender. Various regimes utilized by pain clinics should have been tried first & certainly should be tried now including possible acupuncture. There is also possibility of referred pain. Surgery often does not help with this kind of pain unless it is due to intermittent trusts torsion in which case fixation of testis, to prevent twisting of spermatic cord, takes care of problem.
