Please have him: Seen by his doctor. It is hard to diagnose a rash without seeing it. This could be a reaction to something, but also could be an infection, either viral or bacterial. You didn't mention your nephew's age; if he is an infant, I would be more concerned and call his doctor today. ...Read more

See your MD!: Cellulitis is a very serious infection and if it is spreading you need to see your doctor as soon as possible to evaluate your response to treatment. Even though you are already on Bactrim, (sulfamethoxazole and trimethoprim) I would have your doc re-evaluate your treatment to determine why the redness is spreading. ...Read more

Well, you are not: In the age group where we expect to see herpes zoster or shingles, but that might be what you have. It is difficult to say for sure without a picture (which you could upload to HealthTap for a Virtual Consult). Blistering rash with pain is the hallmark. This: This sounds like a possible cellulitis (skin infection). You will need to be examined by a doctor and get treated. Antibiotics will clear this up if it is in fact bacterial in nature, other possibilities are herpetic lesions, allergic reaction, or even bug bites.

Go and let a dermatologist see these and decide whether further diagnostic studies are indicated or whether he can make the diagnosis clinically. Yes!: Definitely go get checked out in the er. The blistering wounds could be warfarin necrosis. If you have this, the warfarin will need to be discontinued in favor of another blood thinner. With recurring nosebleeds, you should have your protime checked. ...Read more

Maybe HSV, not VZV: Shingles rarely recurs so frequently but HSV does, and recurrent herpes simplex can look identical to zoster. 1) Retest next outbreak to confirm varicella zoster virus (VZV), and PCR test for HSV. 2) Have an HSV IgG antibody test, expecting maybe positive for HSV1. 3) Continue valacylovir to prevent recurrences, whatever the cause.
