Rash: It would be rash to try and diagnose a cutaneous eruption without seeing it. Yours sounds like it may either be a contact dermatitis or an allergic reaction, but there are several diseases endemic to South America which can produce lesions of this nature. My daughter has a rash in between her fingers where the finger connects to the next finger/hand. Lamisil (terbinafine) isnt working so is this not a ringworm?
Maybe: Ringworm/fungal infection is occasionally found between fingers and toes. This could be a fungal infection that is resistant to lamisil (terbinafine). I'm taking terbinafine HCl 250 mg tablet once a day, can I also take fluticasone propionate nasal spray 40 mcg for my flu? Will it be bad together?

I was taking lamosil for 35 days when i started breaking out in a rash and hives. How long will that last? I've been off for 3 wks?
Up to about 3months: Lamisil (terbinafine) can last in the system for a long time because it distributes to skin, nail, central nervous system and even adipose (fat) tissue. It may remain in the system for up to 3 months after therapy is discontinued. Increase water intake to help its elimination.
