Knee, hip, back: If the thigh problem is immediately above the knee this could be a knee problem (cartilage, bakers cyst, joint problem). Knee pain rarely radiates up the leg. Anything higher up is unlikely to be knee. This could also be referred pain from the hip or back. A good physical examination by someone experienced with these problems should give you the answer. ...Read more

OA vs PFPS: Two possibilities are most likely. The first is osteoarthritis, or wear and tear ("rust in the joint"). This is usually in middle-age people but can occur earlier with excessive use. Another option is patellofemoral pain syndrome, or inflammation where the kneecap tracks on the femur (upper leg bone). In most people, these can both be helped with antinflammatory meds and quad strengthening. ...Read more

What exercise modification of a free-weight squat would you prescribe for an individual who experiences knee pain when standing from or sitting in a?

PFPS: You may have what we call patellafemoral pain syndrome. If you get checked out by ortho/sports med you may need pt. If you have pain with stairs, standing up from a seated position, squatting, kneeling all around the knee cap this seems what it could be.

See Doctor: First try nsaids like ibuprofen, aleve and you can even add some tylenol (acetaminophen) to help with pain. Ice the area for 20 min at a time and see if these help reduce the pain. If this persists longer than a few days, consider seeing your doctor for stronger medications to help. Typically most people get better within a few weeks of an issue. Knee problems: Knee pain and stiffness can come from many conditions/ injuries, but with the sensation you describe as "buckling" I would suggest you see a physician to check for ligament or meniscus problems.
