Diabetic: First of all you most likely have type 2 diabetes. If so you are not a diabetic - you have diabetes - and you should be working for the next several years to rid yourself of diabetes.
Diet, exercise, and decreasing medication doses for management of diabetes if where you should be.

Yes: Hi. Metformin and sitagliptin work via different mechanisms, and together are synergistic. So yes, more effective than one drug alone. Note even the combination may not be adequate for achieving tight control. Your doctor will follow your A1c response, and if it's in the mid-low 6's, you're good to go. No. Need Insulin: Type 1 diabetes is a complete lack of Insulin thus Insulin must be given for survival. Symlin (pramlintide) is another injection that can be used with Insulin to treat type 1 diabetes. Amaryl+janumet and other oral medications are indicated for type 2 diabetes (insulin resistance). Janumet (sitagliptin and metformin) is a combo-drug formed by adding sitagliptin (aka, Januvia) and metformin, hence the combo brand name (and combo price tag). It has a BLACK BOX WARNING for potential lactic acidosis which can occur due to metformin accumulation. This is rare, but fatal in 50% of cases, esp. In diabetics w/ kidney disease. No ones perfect/?: I'm not sure I completely understand your?, but as an insulin dep diabetic on Janumet (sitagliptin and metformin) you are on pretty tx. No fixed dose medication will completely control your sugar if you have a high carb meal. Janumet (sitagliptin and metformin) usually does a pretty good job helping. There are ways of adjusting your tx based on carbs. That said no one is perfect and an occasional cheat as long as not crazy high sugar is OK & nl. ...Read more

If type 3 (most common) the best treatment is not janumet (sitagliptin and metformin) (metformin and Januvia) but a type of drug called a sulfonylurea like glyburide.
