An ENT can look in your ear and offer options on whether you could benefit from wax removal vs medication vs should do nothing about it. See your doctor: Its impossible for us to tell what's going on without a physical exam. I'd see your doctor to have a look. Amongst other things - it could be an otic infection, impaction or foreign body which has become lodged in there. I'd suggest not using the Q-Tips as this might only exacerbate any problem.
