Mono: Spleen becomes: Palpable when the size gets double. Most of the time doctors don't feel it- though enlarged. Ultrasound detects the enlargement, but not done routinely. Similar but: Both cause very sore throat with swelling and fever. Both can be diagnosed readily with a rapid test. Mono rapid test is less accurate. Mono significant systemic symptoms such as fatigue, sleepiness, nausea, and the swelling tends to be more severe and more dramatic appearing.
