Is an ongoing pleural effusion in an elderly nonambulant person a potential "breeding ground" for an infection? If so, is the risk insignificant?
In general, no: A pleural effusion can be what is called an empyema which is actually from an infection and is therefore infected. If the effusion is passive, like heart failure it usually does not become infected though potentialicould get infected. In the second case it is not common. ...Read moreSee 1 more doctor answer

Yes: Antibiotic ear drops can be prescribed to treat outer and middle ear infections even with a perforation. Sometimes oral antibiotics are needed for middle ear infections alone or with the drops. The other pressing issue is the "holes" you were born with. That is unusual and you should see an ENT doctor for full exam and hearing test. ...Read more
How serious is a 81% oxygen saturation reading on room air combined with a po2 of 46 mmhg? Could it be an indication that death is imminent in an elderly person?
Yes: An oxygen saturation, the % of o2 in the blood generally is >95 in a healthy person, other illnesses may cause that to change. End stage disease(of any kind) can lead to low o2 levels as a natural part of the dying process. As long as the person is not experiencing any discomfort and the goal is comfort only- checking o2 levels may not be indicated and only more distress to the caregivers. ...Read moreSee 1 more doctor answer

Why acute otitis media is dangerous in adults? Could it be a sign of cancer or weak immunity? Thanks
??Dangerous ??: I would consider it unusual and a nuisance but not dangerous. If it led to a perforated ear drum, those do take much longer to heal and may require surgery. Adults who get acute otitis generally did so as kids, and many have allergic and or sinus issues that predispose to the problem. I can't buy a cancer or immune defect as a significant factor. ...Read more

Is there a high risk of hearing loss in canal occlusion surgery for bppv vertigo? Is it total hearing loss or only some?
Probably no loss: Bppv is the result of loose otoconia (ear stones) in the inner ear. This disease as its name suggests is benign. If you have a dehiscence of a canal (superior semicircular canal), that is a more serious problem and is not bppv. However, the surgery to occlude a canal should not cause a hearing loss. The location of the canal is not that close to the cochlea. However, there is always a risk. ...Read more
