Can take 1-2 weeks: But call your pediatrician with your concerns. This is a sign that the treatment isn't working, and it may be something else.

Check with pediatric: Do not apply an antifungal cream without a diagnosis of fungal infection. You can use a small amount of barrier cream such as Desitin (zinc oxide diaper cream) or Calmoseptine- not antifingal mediciation until patient is seen and evaluated.

White Facial Spots: White facial spots may be postinflammatory hypopigmentation meaning that irritation of the skin can cause a decrease in pigment. Treatment would be to decrease any irritation and the use of Elidel (pimecrolimus) cream a prescription.

Bleeding rash?: Candida usually causes a red, 'heaped up' skin rash with 'satellite' lesions. If there is bleeding associated with this rash, especially involving a baby, it needs to be seen and evaluated to assure that it is not a poly-microbial infection, allergic reaction or combination or the above with chemical irritation from urine. Call your pediatrician for assessment and or examination of the rash.

Yes&yes but only if: Lotrimin is an over-the-counter brand of clotrimazole which is an antifungal. While Lotrimin doesn't have specific adult vs pediatric guidelines, clotrimazole is approved for use in children. However, it's best to have your infant evaluated to confirm your suspicions. Lamisil is an OTC brand of terbinafine which is also an antifungal but only approved topically for children older than 12yo.

A: A rash that has been treated with all these therapies is very difficult to make suggestions for further therapies without seeing the rash. I would have expected a typical diaper dermatitis due to yeast to have cleared with the ketoconazole. I would be hesitant to use a stronger steroid under a diaper. I would suggest making an appointment with your pediatrician or a dermatologist. Emergency room doctors are generally not use to treating diaper rashes. Rashes just have to be seen to diagnose accurately and treat appropriately.
