I am a 40 yr old with stage IV COPD my fev1was 0.95 or 23%. Is that bad for my age...I smoked a pack a day for 20 years...?

Not good!: Stage IV isn't good. I suspect that you're short of breath w/any activity, possibly even at rest, given your numbers. I hope you've already quit smoking. Are you taking your medications/inhalers as directed? Inhaled corticosteroid? Long acting beta agonist? Anticholinergic? Maybe even pde4 inhibitor? Vaccinations up-to-date? Pulmonary rehab & supplementary oxygen might be helpful, too. ...Read more

See below: I would recommend having a follow up with your pulmonologist. You are on good medications. However, if you are having a flare up, you may need more acute treatment. If you are not, then you may need other testing done, such as an ECHO or maybe try pulmonary rehab. I would be happy to discuss further if you need more information. Hope you feel better. ...Read more

Associated with chronic inflammation of the lungs to noxious particles or gases like smoking. Most important risk factor for COPD is cigarette smoking. Amount/duration contribute to disease severity. I have copd, was put on oxygen due to a bad cold and low oxygen levels. Why am I short of breath while on this oxygen? It's set at 2.

Several Factors: Your shortness of breath can be due to inadequate oxygen getting into your blood. This is a function of gas exchange.
It might be manageable by increasing your oxygen, but you should check with your doctor first. Sometimes increasing the oxygen can actually make you breathe in and out less, resulting in the retention of carbon dioxide - which is bad.

Good luck and feel free to ask more. ...Read more

How is COPD confirmed? I'm 27, have asthma, have smoked in the past, and after a bad chest infection 5 months ago my lungs have still not recovered.

Pulm Function Test: Pulmonary function testing. It would be very unusual for you to have COPD at age 27. Given your history of asthma, your lungs are more susceptible to infections and other insults.

Usually to have COPD we consider the equivalent of 10 years of smoking one pack per day and being age 40 and older.

Did you have a follow-up chest x-ray after your pneumonia? I would recommend to see a pulmonologist. ...Read more

See neurologist: This is a difficult situation. It sounds like you have had some work-up for lupus which is appropriate. It sounds like this is a diffuse problem. When you speak about progressive muscle weakness, you may consider seeing a neurologist if you haven't already. There are diffuse myopathies that may occur from diseases that affect the nerves and muscles. ...Read more
