Ovarian cancer: Ovarian cancer commonly spreads by the cancer cells sloughing off the ovary, floating in the abdominal cavity, and planting in other areas, especially where the fluid pools. The bladder surface is one of those areas where implants commonly occur, and can cause blood in the urine. Sometimes the ovarian tumor itself can rest on the bladder and cause bladder irritation, and potentially bleeding.

Cancer spread: Unfortunately, ovarian cancer is usually not detected until it has already spread and involved other organs. If it has involved the bladder you will usually see blood in the urine.

Talk with your doc: As a gynecologic oncologist, I don't usually follow estrogen as a marker for ovarian cancer. There are a few rare exceptions (granulosa cell tumors). Estrogen is also made in fat cells. You and your wife should have a conversation with her doctor about why they are sending estrogen levels at all. Aromatase inhibitors are a group of drugs that prevent estrogen production and can be used to treat.

Backward: There is no screening test for ovca. However, if you have ovarian cancer, a ca-125 level may or may not be elevated. If it is, it may be one thing monitired for response. Getting ca-125, an expensive blood test of unproven value before knowing you have the cancer is inded backward, costly, non-specific and can led to unnecessary worry.

Yes that would be th: Spread into kidney is uncommon. It may occur by surface implantation when there is intra-abdominal tumor all round the kidneys or it could spread via blood although that is a less common route of spread for ovarian cancer compared to most other cancers.

Ovarian cancer: More often than not, ovarian cancer does not have symptoms. If your scan and blood tests are negative, obviously one must consider other causes. Wasn't sure what you: Meant by low counts, but I would not leap from low blood count to ovarian ca. Ovarian ca does not tend to metastasize to bone marrow, where blood cells are made. Contralateral ovary, fallopian tubes, peritoneum, liver, lungs, bone and brain are the most common sites. I think the more intriguing question is why you are making the connection? Is there a family history that concerns you?

NO: There is no good / reliable blood test for ovarian cancer as a screening test. However, if you have ovarian cancer, a cancer marker- ca-125, in majority times is elevated in ovarian cancer and this can be used to monitor response to treatment.

No: Some may offer a ca-125 blood test as a screening for ovarian cancer. Unfortunately, while ca-125 is found in 80-90% of ovarian cancer patients, it is not very specific (meaning it is elevated in a number of conditions) and is not found in early cancers. It is better used as a method of following cancer treatment and progression, once it is diagnosed.
