Natural response: 38 degrees celsius is not a particularly high fever, though it might be significantly higher if you were not giving him paracetamol. Paracetamol reduces fever but does not eliminate it. Fever is the body's natural response to infection and helps the immune system fight an infection. Temp: The body temperature may be only one small part of the defense mechanism against invading organisms. If the fever is high, you will feel better treating that, and it won't alter the course of your body fighting the infection. I don't think this issue warrants a visit to the emergency room. However, an office visit with his pediatrician would be a good idea. His doctor will be able to examine him and determine if there is a source for infection that requires antibiotics (which could possibly be an ear infection). In the meantime if tylenol (acetaminophen) is not working for his pain, children's Ibuprofen may provide some relief. Check with the pharmacist for the correct dose if he is under 2 years of age (do not use if he is less than six months old).

generally after shots the fever should respond and should last only 24hrs.
you could try ibuprofen 10 mg /kg every 6 hrs but if fever is still not responding in 24 hrs baby needs to be checked

How is your child acting and responding to you ; Is child very lethargic, and not acting "normal"? This could be an emergency.
One objective is try to control fever to prevent febrile seizures.
If child is fine, you might want to try pediatric ibuprofen in between Tylenol (acetaminophen). How often are you giving Tylenol (acetaminophen)?
