If it wasn't beforehand, there may be some traction/tension on the deeper tissues that are pulling the nipple into the breast mound. If this is something: ...new and the pain is continuous, rather than cyclic, the prudent thing would be to get a mammogram. The nipple sticking to your breast tissue when you lean forward is an unusual finding. Especially if this worries you, bring it up with your GP. Normal: Normal to have inverted nipples. Common in women as well as men.
Not sure what HV refers to but 'saggy' breasts could be due to size or normal genetics. If it's a cosmetic issue you could address it with a plastics doctor.

Pregnancy will make your breasts fuller but generally with age and after pregnancy your breasts may develop stretch marks or lose more fat so might be worse, hard to say. ...Read more
